A problem has recently shown up with Amazon Fire tablets where the
Install button is greyed out on the app permissions screen when installing an APK file created with MIT App Inventor. This workaround appears to be effective:

Begin installing the app from the QR code as normal, until you
get to the permissions screen where Install is greyed out.
- Press the power button to put the device to sleep.

Press the power button again to unlock the device.

The Install button should now be activated.
The problem occurs with FireOS 5.6 which was released recently, so
if your tablet has not yet received the update it may be working
fine at the moment. The bug is reported to have originated in the
October security patches for Android 5.1 Lollipop on which FireOS
5.6 is based, so it may also affect some Android devices.
Update: Amazon pushed out a new build of FireOS 5.6 in early December which fixes this bug.
